Deluxe Physical Therapy is an in-network provider with Original Medicare Part B and out-of-network provider for all other insurances.

For non-Medicare patients, upon request we will provide you with the necessary documentation, e.g., a superbill, to submit a claim to your insurance provider. Please check with your insurance what your out-of-network benefits are.​

Add-on Services to treatment sessions for Medicare Patients (not covered by your insurance) may include dry needling, therapeutic massage, and other.
